Major changes to comment system

Over time its been increasely annoying seeing the comment system trying to track where comments are. Each time a new section is added, countless more code is added to try and link to the comment.

I have now changed the code. The comment system now saves the url and name of the page. This means each comment can be tracked and linked directly to it. The recent comment page now includes the correct title. I have hopefully updated all pages that use the comment feature, however if you see one saying something about missing db_name, let me know and ill update it!
